Examples of High Risk Complaints include, but are not limited to, allegations of:

unsafe care or serious medication errors causing harm
breach of duty of care resulting in hospitalisation or death
systemic issues affecting multiple individuals
breaches of privacy or confidentiality
physical or environmental hazards that pose a serious risk to the safety of residents, staff, or visitors.
fraud, corruption, or financial misconduct, such as falsifying records, misusing funds, or dishonest reporting
staff misconduct, including abuse of power, conflicts of interest, or unethical behaviour.
If you select “Yes” your identity and personal information will be collected by Estia Health, and used for the primary purpose of managing and responding to your feedback or complaint. Your information may be disclosed to those within Estia Health involved in handling the matter, such as relevant senior managers or other authorised employees.
We will take all reasonable steps to protect your identity, and your personal information will not be used or disclosed for any other purpose without your consent, unless required or authorised by law (for example, where there is a serious risk to health, safety, or wellbeing).
